Patient Family Engagement for Equity (PFEE) involves actively including patients and their family caregivers in health care decisions—from bedside care to organizational operations—to ensure that care is tailored to diverse needs and backgrounds. PFEE is an active strategy to improve outcomes, including an individual's health outcome or organizational outcomes, such as reducing Readmissions. When people with lived experience share insights, we begin to learn about the causes of what we see in the data. The data tells us what is happening, patients and family caregivers tell us why it's happening. We often refer to it as the “secret sauce” of quality improvement for this reason.

Ready, Set, Go! was co-designed by hospital staff and Patient Family Partners in partnership with HQIC to address and connect the critical processes that take place between admission planning, bedside rounding, and discharge planning in an effort to reduce hospital readmissions. This new resource includes key insights into the three processes plus five embedded tools to enhance current processes at the hospital.
These three activities will assist you improve patient outcomes and avoid readmissions, and also includes implementation guidance to support hospitals ready to take action.

We bring experience engaging patients and family caregivers in highly technical collaborations – including research and measure development.

Monthly Think Tanks
Connect with your colleagues in the Covergence HQIC PFEE Think Tank. Each 30-minute Think Tank session will address a specific PFEE topic. Participants will have ample opportunity to ask questions and learn from peers. These monthly Think Tanks are open to hospital staff who would like to learn more and grow PFEE practices at the hospital.
